Fulltime Case Manager required to join the Brotherhood of St Laurence Community Aged Care team located at the Sunshine office. This role involves consulting with consumers, carers and service providers to provide an integrated high-quality service to meet the consumer care plans and requirements.  

Critical to the success of this role is the Consumer Directed Care Model (CDC) a person-centred, customer focused and active service framework to maximise consumer wellbeing and choice whilst maintaining an allocated caseload to a high standard. Brotherhood’s model assumes all consumers have the ability to self-manage their home care package.

In this role you will be:

  • As the primary point of contact for consumers, carers and service providers provide information on services, support with health issues related to disability, ageing and dementia.
  • Conduct, consumer assessments including planning, monitoring and regular reviews using a person centred and reablement approach to support options to assist clients that meet their needs and aspirations within available resources
  • Support and respond to consumers and carers issues/concerns taking appropriate actions to resolve and escalating matters to the Regional Team Leader – HCP where appropriate in a timely manner
  • Experienced in maintaining an allocated Case load to a high standard
  • Locate and arrange customised services required by the carer and consumer as per the agreed care plan liaising with internal and/ or external stakeholders as needed

   Skills and Experience required for the role:

  • Relevant tertiary qualifications in Nursing (Division 1), Welfare, Occupational Therapy, Psychology, Disability, Psychiatric Nursing, Physiotherapy, Social Work or other related discipline.
  • Experience in person centred case management and a reablement approach in the aged care and/or disability sector including support to people with dementia.
  • Significant experience in the development of person-centred planning and the ability to demonstrate a clear understanding of case management, assessments, networking and advocacy
  • Excellent team player with ‘one team’ approach to achieve team goals
  • Outstanding communication skills to engage with a diverse range of clients and their carers
  • Exceptional customer service focus
  • Knowledge and competency in Microsoft Office Programs
  • Current Victorian Drivers Licence.

The Brotherhood provides a workplace that values and support their staff and provides ongoing training and development. 

Salary: $74,531.95 to $80,194.32 plus superannuation and company car with full private use.  Salary packaging available
