About the role
We are seeking an experienced Community Support Worker who can provide in-home care services to aged individuals who require support to live independently, and their carers. Our in-home services promote a wellness and reablement approach to care, through the delivery of home-based activities of daily living support to clients that promote health and wellbeing.
Salary: $70,393.91 per annum or $35.62445 per hour plus 25% casual loading, based on skills and experience plus 12% superannuation (pro-rata for part-time employment)
Key Responsibilities

Provide support for clients in activities of daily living, to promote autonomy, wellness and enablement goals.

Support carers by providing meaningful and enjoyable experiences to a client, that meet the needs of the client, as documented in the Care Plan.

Apply the Consumer Directed Care and wellness and reablement approaches.

Maintain client privacy and confidentiality.

Provide transport to clients as per their Care Plans to meet health and wellbeing home based goals.

With guidance from the Program Lead, implement various initiatives or activities to prevent or manage challenging client behaviours.

Key skills and experience

Certificate III in Individual Support or equivalent, or working towards obtaining this

Holds a current Victorian driver’s license
- Knowledge of and ability to participate in quality improvement activities and strategies
- Client-centered approach
- Highly developed interpersonal and communication skills, with a demonstrated ability to work with and engage individuals from diverse backgrounds
- Strong organisational and time management skills
