Community Support Worker - Home Care

We’re looking for a professional Community Support Worker with relevant experience in home care services to aged care individuals who wants to join our team on a casual basis, working based in Frankston City Council and Casey City Council and surroundings.

Location: South East - Frankston 

Job Type: Casual 

Salary: $63,797.69 per annum, based on skills and experience plus 12% superannuation. (pro-rata for part-time employment)

About the role

  • Provide support for clients in activities of daily living, to promote autonomy, wellness and enablement goals.
  • Provide transport to clients as per their Care Plans to meet health and wellbeing home based goals.
  • With guidance from the Program Lead, implement various initiatives or activities to prevent or manage challenging client behaviours.
  • Develop and maintain positive lines of communication with families, advocates and clients, presenting a professional image at all times.
  •  Complete and maintain accurate client records, including but not limited to Care Plans, assessments and note

What we are looking for

To be successful in this role, we are looking for someone who has the following key skills and experiences:

  • Certificate III in Individual Support or equivalent, or working towards obtaining this
  • Holds a current Victorian driver’s license.
  • Specific work requirements include work-based travel and attendance at a variety of different work locations
  • Proof of eligibility to work in Australia is required
  • A Police Check is required for this position. The Brotherhood will facilitate this process
  • A Statutory Declaration to confirm past criminal history is required as per the Aged Care Act is required - BSL will facilitate this process
  • A Working with Children Check is required for this position. The Brotherhood will facilitate this process
  • A NDIS Worker Screening check is required for this position. The Brotherhood will facilitate this process
  •  Aged Care Banning Orders Register checks shall be undertaken
