THE ROLE
This Case Manager consults and works with consumers and their carers to provide integrated high-quality services in accordance with funding and legislative requirements.
- Be the primary point of contact for consumers, carers for information on services available and support with health issues related to disability and ageing, including dementia.
- Support and respond to consumers and carers issues/ concerns taking appropriate actions to resolve them and escalating matters to the Regional Team Leader – Home Care Packages where appropriate in a timely manner.
- Generate and provide information and reports as requested, including handover notes regarding consumers and ensure assessment tools and data entry is completed with all consumer information in a timely manner.
Salary will be in the range of $44.47716 to $47.85696 per hour plus 25% casual loading, based on skills and experience plus superannuation.

To be considered for the role and be successful in it, we’re looking for someone who has the following key skills and experiences:
- Minimum 12 months of demonstrated experience in person-centred case management and a reablement approach in the aged care and/or disability sector, including provision of support to people with dementia.
- Demonstrated knowledge and competency in Microsoft Office Programs including Word, Excel and Outlook.
- Ability to effectively communicate and explain information to internal and external stakeholders
- Relevant tertiary qualifications in Nursing, Social Work, Occupational Therapy, Physiotherapy, Psychology, Welfare, Disability or other related disciplines.
- Understanding of and empathy with the values and ideals of the Brotherhood of St Laurence
