Program Lead - Social Connections and Respite

The Brotherhood of St Laurence Social Connections and In-Home support program has a vacancy for a Program Lead based at Frankston. This program provides social support, wellness and reablement, recreational and educational support and one on one person centred care services to clients within the southern-metropolitan region. As the Program Lead you will assist the staff to provide social support activities and one on one individualised personal centred care that supports the clients.   

The Program Lead is an ongoing full-time role working Monday to Friday 8.30 am – 5.00pm.

To be successful in this role you will have extensive experience in Aged or Community Care excellent leadership and management skills to support staff and clients.  You will be a natural at building rapport and relationships with clients form very diverse backgrounds.  Knowledge of government funding is essential to the role.

Candidates will require:

  • Extensive experience working with aged care clients with complex care needs including dementia
  • Strong customer service focus undertaking regular consultation with clients and their families form diverse backgrounds
  • Proven experience in leadership, managing and developing staff, ensuring a ‘one team’ approach
  • Proven experience in Program design, delivery and evaluation.
  • Highly effective interpersonal, multi-tasking and communication skills with the ability to build effective relationships with stakeholders
  • Well-developed organisational and time management skills, ability to plan workload, prioritise and meet deadlines
  • Understanding of the My Aged Care, Commonwealth Home Support, Home Care packages funding models. 
  • Proven ability to identify and resolve basic problems and make appropriate recommendations.
  • Understanding of and empathy with the values and ideals of the Brotherhood.

This is an ongoing full-time position working 5 days per week (Monday – Fridays 8.30 am to 5.00 pm).   The Brotherhood provides a workplace that values and supports their staff, offers career development opportunities, a competitive salary and salary sacrifice options.

Salary:  $85,264.31 - $89,124.91 per annum based on skills and experience plus superannuation. Attractive salary packaging provisions are available to increase take home pay.
